Tom & Jerry 3, "Wait and See!," and the infamous Felix the Cat bootleg, (with an even worse Game Over screen) for starters....
Wait and See!'s was also sorta disturbing, but not as visceral.

I just googled the Felix the Cat bootleg and I just laughed at how twisted and demented it was. Like, holy hell, whoever is behind those game over screens has some bizarre gore fascination going on with beloved children's characters.
